How the platform works

The architecture is designed to do the continuous work of care supervision at population scale. Signals from the home, the care ecosystem, and the broader health system flow into DT4H™ and HealthGraph™, where they are interpreted in context and converted into next-best actions through Ibis™.

In practical terms, the loop works like this:
