How the Operating System works

Signals from the home, clinical systems, payer systems, and the broader care ecosystem continuously update the member’s Digital Twin for Health and the connected HealthGraph.

The Operating System runs a continuous loop:

  • Maintain Persistent Understanding: continuously update the member’s DT4H as new information arrives.
  • Detect Meaningful Change: recognize changes that may require attention before they become avoidable deterioration or utilization.
  • Determine Who Should Do What: determine who should do what, when, why and how to keep care moving.

This closed loop is what makes continuous care operational. The member remains the organizing unit, while different members of the Care Circle step in and step out as needs change.
