The Clinical Innovation Council sits at the center of that process. It helps ensure that the platform, the clinical model, and the Care Continuity Team™ evolve together—so technology can carry more of the continuity burden safely, while human teams stay focused on the moments where judgment, compassion, and safety oversight matter most.

Reviews patterns surfaced by the HealthGraph™ to refine how chronic conditions are monitored and proactively managed across the population.
